Why I Don’t Recommend Policosanol Cholesterol-Lowering Supplements
According to cholesterol-lowering guidelines, dietary modifications are the cornerstone for controlling the circulating concentrations of LDL cholesterol. As the conversation around cholesterol management evolves, many individuals turn to dietary supplements, commonly referred to as nutraceuticals, available online or over-the-counter. However, the evidence supporting many of these supplements is often limited and debatable.
The Case of Policosanol
One supplement that has garnered attention is policosanol, a waxy extract derived from sugarcane. Initial studies reported an impressive 24% reduction in LDL cholesterol compared to a placebo, making it appear more effective than phytosterols and rivaling statin drugs. Given these results, one might wonder why I do not recommend policosanol, especially since it reportedly works more than twice as well at a much smaller dose and is more cost-effective.
Inconsistencies in Research
Over 50 studies have suggested significant reductions in cholesterol levels with as little as 2 mg of policosanol. In contrast, to achieve less than half the effect, one would need to consume 2 grams of phytosterols. However, it is crucial to note that almost all studies demonstrating these substantial reductions were sponsored by the Cuban enterprise marketing policosanol. When independent researchers attempted to replicate these positive outcomes using different formulations, they found little to no effect.
Independent Studies and Results
In one notable independent study that tested the authentic Cuban policosanol preparation, results were disappointing. Even at a typical dose of 10 mg, and up to eight times that amount, the reduction in LDL cholesterol was minimal—between 2% and 8%. Interestingly, the placebo group also experienced an 8% drop, indicating that the results were not statistically significant. Hence, policosanol's efficacy as a cholesterol-lowering agent has been called into question.
Understanding Meta-Analyses
For those unaware, a recent meta-analysis may suggest a significant drop in LDL cholesterol on par with statin drugs. However, it is essential to scrutinize subgroup analyses.
